Exemestane, sold under the brand name Zionova Aromasin among others, is a medication used to treat breast cancer. It is a member of the class of antiestrogens known as aromatase inhibitors. Some breast cancers require estrogen to grow. Exemestane is a steroidal aromatase inhibitor which prevents the production of estrogen.

Mechanism of action

Exemestane is an irreversible, steroidal aromatase inhibitor (type II). It is the first steroidal aromatase inhibitor to be marketed.

It works by binding permanently to the aromatase enzyme, which is responsible for the conversion of androgens to estrogens.

Exemestane has approximately 1000 times the binding affinity to the aromatase enzyme than tamoxifen and approximately 10,000 times the binding affinity of anastrozole.

Without the enzyme, the conversion of androgens to estrogens cannot occur. Exemestane is therefore presented as a very effective treatment for breast cancer.

Exemestane inhibits the estradiol production in the adrenal glands, which eventually leads to a decrease in ovarian estradiol production.

Exemestane also induces apoptosis, arrests cell cycle progression at the G2/M phase and dramatically inhibits the growth of human breast cancer cells in vitro.

INDICATIONS

Zionova Aromasin may be used to treat advanced breast cancer in postmenopausal women.

It may also be used to treat advanced breast cancer in postmenopausal women with hormone-receptor positive, HER2-negative breast cancer who have received at least one chemotherapy treatment.It may also be used to treat hormone-receptor positive, HER2-negative advanced breast cancer in postmenopausal women who have received at least two chemotherapy treatments.

DOSAGE

Zionova Aromasin is available in 20 mg tablets, which are to be taken once daily.

Aromasin is usually taken on an empty stomach around the same time each day.

Aromasin may be taken with or without food.

The dosage of Aromasin is based on the medical condition being treated, age, weight, laboratory tests and response to treatment.

Store at room temperature away from moisture, heat and light. Keep out of reach of children.
