Human chorionic gonadotropin (hCG) acts like LH, the signal that tells the testes to make testosterone and sperm. It is a fertility medicine supplied as freeze-dried powder.

How it works
hCG binds LH receptors in the testes, so they keep making testosterone and sperm even when your own LH is switched off.
Half-life: about 24-36 hours.

How levels rise and fall after a single dose, based on a half-life of about 24-36 hours. Each mark on the axis is one half-life; after about five, almost none is left.

Clinical reference dose
Clinical reference: Canadian labels for male hypogonadism use about 1,000–4,000 IU two to three times a week. Doctors adding it to TRT to preserve fertility commonly prescribe 250–500 IU two or three times a week.
Mixing: adding 5 ml of bacteriostatic water to this 5,000 IU vial gives 1,000 IU per ml. On a U-100 insulin syringe, 10 units (0.1 ml) then holds 100 IU.
High doses raise estradiol and can desensitise the testes.

Injection sites
HCG 5000IU comes as a freeze-dried powder. HCG is labelled for both routes: intramuscular (the original Pregnyl label) and subcutaneous (the Ovidrel label). Most people now use subQ with an insulin needle, shown below.

Finding it. Anywhere on the belly at least 5 cm (2 in) from the navel, avoiding scars, stretch marks and the belt line. Pinch a fold of skin and inject into the fold.
Why people choose it. The most-used subcutaneous site: the most fat, easy to see and reach, and absorption is steady. It is the first site listed on the labels of weekly GLP-1 pens.
Watch out. Move each injection at least 2–3 cm from the last one to avoid lumps (lipohypertrophy).

Why these sites for HCG 5000IU
Subcutaneous (subQ) or IM- Belly first. The abdomen has the most fat and the steadiest absorption, and you can see what you are doing. The thigh and back of the arm are there to rotate into.
- Small volumes, short needle. SubQ injections are small (around 1 ml or less per spot). An insulin syringe (4–13 mm) reaches the fat without touching muscle; pinch the skin if you are lean.
- Rotate every time. Move each injection 2–3 cm from the last. Repeating one spot causes hard lumps under the skin that absorb unevenly.
Every time: wash your hands, wipe the vial top and the skin with alcohol and let it dry, use a new sterile needle and syringe, and never share.
