Anadrol (oxymetholone) and Dianabol (methandrostenolone) are both oral steroids known for fast weight and strength gains. Anadrol is generally the stronger and harsher of the two. Both are hard on the liver and raise blood pressure and cholesterol.
- Much of the early weight gain from both is water and glycogen.
- Both are 17-alpha-alkylated, which means liver strain.
- Short use with bloodwork and liver monitoring is the standard harm-reduction advice.
These two orals have been compared in gyms since the 1960s. They are often described as interchangeable, but they differ in strength, side effects and history. Here is a straightforward comparison.
At a glance
| Anadrol (oxymetholone) | Dianabol (methandrostenolone) | |
|---|---|---|
| Original medical use | Anemia, wasting from HIV | Muscle building in the 1950s–60s; withdrawn |
| Approximate half-life | 8–9 hours | 3–6 hours |
| Weight and strength gain | Very fast | Fast |
| Water retention | High | High |
| Estrogen-type effects | Yes, despite not converting to estradiol | Yes, converts to a potent estrogen |
| Liver strain | High | High |
| Blood pressure | Raises markedly | Raises |
Anadrol (oxymetholone)
Oxymetholone was developed by Syntex in the 1960s and is one of the few oral anabolic steroids still prescribed in some countries, mainly for anemia caused by bone marrow failure. It strongly stimulates red blood cell production. Studies in people with HIV wasting and in older men showed significant gains in lean mass and strength, along with clear rises in liver enzymes.
Oddly, oxymetholone does not convert to estradiol, yet it can cause gynecomastia and water retention. It may act directly on estrogen receptors.
Dianabol (methandrostenolone)
Dianabol was developed by Ciba in the late 1950s with the help of a US team physician, Dr John Ziegler, specifically to build muscle in athletes. It was later withdrawn from human medicine. It converts to methylestradiol, a potent estrogen, which explains its water retention and gynecomastia risk. Read our Dianabol guide for more detail.
Shared risks
Liver
Both are 17-alpha-alkylated: a methyl group protects them from being broken down by the liver when swallowed, which is what makes them work orally but also strains the liver. Effects range from raised liver enzymes to cholestasis (blocked bile flow) and, with long or heavy use, liver tumours. See liver support during orals.
Heart and blood pressure
Both can sharply lower HDL cholesterol and raise blood pressure within weeks. Checking blood pressure at home is the simplest daily check.
Suppression
Both shut down natural testosterone production quickly.
Blood pressure: the risk people underestimate
Liver damage gets most of the attention with orals, but blood pressure is the problem many people notice first. Both Anadrol and Dianabol cause the body to retain sodium and water, and blood pressure can rise noticeably within one to two weeks. High blood pressure usually causes no symptoms, so the only way to know is to measure it. A validated upper-arm monitor from a Canadian pharmacy costs about the same as a month of supplements. Hypertension Canada recommends measuring twice in the morning and twice in the evening, seated and rested, and averaging the readings. Repeated home readings at or above 135/85 mmHg are considered high. Cutting back on salty processed food and alcohol helps, but readings that stay high need a doctor.
Why the weight gain is partly water
Both orals cause rapid gains on the scale in the first weeks, often several kilograms. A large part of this is water and stored carbohydrate (glycogen) in muscle. Some of the weight falls away when they are stopped. Realistic expectations help; see realistic first-cycle results.
Bloodwork
Before and during use, test liver enzymes (ALT, AST, GGT, bilirubin), a lipid panel, a complete blood count, kidney function, and estradiol. Your doctor can order these, or you can pay at LifeLabs or Dynacare. Our bloodwork guide explains timing.
Which is “better”?
Neither is a beginner compound. If the question is which is gentler, Dianabol is usually considered somewhat milder than Anadrol at commonly discussed amounts, but both carry the same categories of risk. Harm-reduction resources consistently suggest short use, avoiding alcohol and other liver stressors, and monitoring blood pressure and liver enzymes. For how orals fit into a broader plan, see using orals as kickstarters.

Frequently asked questions
Is Anadrol stronger than Dianabol?
Generally yes, milligram for milligram the effects and side effects of Anadrol are considered stronger, though oxymetholone tablets are also usually higher strength.
Are Anadrol and Dianabol hard on the liver?
Yes. Both are 17-alpha-alkylated and can raise liver enzymes and cause cholestasis. Liver tests before and during use are important.
Why do I gain weight so fast on Dianabol or Anadrol?
Much of the early gain is water and glycogen stored in muscle, along with some real muscle growth.
