Tamoxifen has been prescribed for breast cancer since the 1970s, which makes it one of the best-understood drugs in the world. It also has a long history in men’s health, which is why it shows up in almost every conversation about recovering after a steroid cycle. Here is what it actually does, what the evidence says, and what to watch for.
How tamoxifen works
Tamoxifen is a selective estrogen receptor modulator, or SERM. It sits on estrogen receptors and either blocks them or partly activates them, depending on the tissue. In breast tissue it blocks estrogen, which is why it treats estrogen-sensitive breast cancer and why it reduces gynecomastia (male breast growth).
In the brain, it blocks estrogen’s “slow down” signal to the pituitary gland. The pituitary responds by releasing more luteinizing hormone (LH) and follicle-stimulating hormone (FSH), which tell the testes to make testosterone and sperm. That is the basis for its use after steroids, when natural production has been switched off.
Medical uses and doses
Tamoxifen’s active form builds up slowly. It has a long half-life of several days, so levels take a few weeks to stabilise, which is why it is taken once a day rather than in complex schedules.
Nolvadex after steroids
When you stop testosterone or other anabolic steroids, your brain needs time to restart LH and FSH. During that gap testosterone is low, and people feel tired, flat and low on libido. Some doctors prescribe tamoxifen or clomiphene off-label to shorten the gap, often alongside HCG in men who want to preserve fertility.
The evidence for this specific use is mostly from small studies and clinical experience rather than large trials, and recovery speed varies a lot between people. Side effects
- Hot flushes and sweating, the most common.
- Blood clots. Tamoxifen raises the risk of deep vein thrombosis and pulmonary embolism. The risk is small but real, and higher in smokers and after surgery or long flights.
- Visual changes, rarely, including cataracts with long-term use.
- Liver enzyme changes.
- Mood changes in some people.
Drug interactions
Tamoxifen is converted into its active form by the liver enzyme CYP2D6. Strong CYP2D6 blockers, including the antidepressants paroxetine and fluoxetine and the drug bupropion, can reduce how well it works. Tell your doctor or pharmacist about every medication you take. It also interacts with warfarin, increasing bleeding risk.
Legal status in Canada
Tamoxifen is on Health Canada’s Prescription Drug List. It is not a controlled substance, so possession is not an offence, but pharmacies dispense it only with a prescription.
