Testosterone is partly converted into estradiol, the main estrogen, and men need some of it for joints, libido, mood, bones and cholesterol. Problems come from too much or too little, not from estrogen itself.
- High estradiol can cause water retention, sore nipples and gynecomastia.
- Crashed estradiol causes joint pain, low libido, fatigue and low mood.
- Ask for a sensitive estradiol test, and treat the number and symptoms together.
Estrogen management is one of the most misunderstood parts of using testosterone. Online advice often treats estrogen as an enemy to be crushed with an aromatase inhibitor from day one. That approach causes as many problems as it solves. The goal is balance, guided by bloodwork and how you actually feel.
Where estrogen comes from
An enzyme called aromatase, found in fat tissue, the brain, testes and elsewhere, converts some testosterone into estradiol (E2). More testosterone means more raw material, so estradiol usually rises when you take testosterone. Higher body fat means more aromatase, so leaner people often convert less. Some compounds, like Dianabol, convert to a more potent estrogen, while others such as nandrolone and trenbolone have their own hormonal effects. Oxandrolone, Winstrol and Masteron do not aromatize at all.
Why men need estrogen
- Libido and erections: both depend partly on estradiol.
- Joints and tendons: estradiol supports collagen; very low levels cause aches and cracking joints.
- Bones: estradiol is the main hormone protecting bone density in men.
- Cholesterol and heart: crushing estradiol worsens lipids.
- Mood and brain: low estradiol is linked with low mood and anxiety.
Symptoms of high and low estradiol
| Too high | Too low |
|---|---|
| Water retention, puffy face | Joint pain and dryness |
| Sore, sensitive or swelling nipples | Low libido, weak erections |
| Gynecomastia (breast tissue growth) | Fatigue, flat mood, anxiety |
| Higher blood pressure | Poor sleep, dry skin |
| Mood swings, emotional sensitivity | Worse cholesterol |
Notice that several symptoms, like mood changes and low libido, appear on both sides. That is why guessing does not work and a blood test does.
The blood test to ask for
Standard estradiol tests were designed for women’s higher levels and can be inaccurate at the low levels found in men. Where possible, ask for a sensitive estradiol test (often done by LC-MS/MS). Canadian labs report estradiol in pmol/L. Many healthy adult men sit somewhere around 70–150 pmol/L, but ranges vary by lab, so compare with your own baseline and the lab’s reference range. Test alongside total testosterone, at the low point before your next injection, after levels have stabilised for several weeks.
LifeLabs and Dynacare can run these with a doctor’s requisition or as private tests. See our Canadian bloodwork guide.
First-line fixes that are not drugs
- Lower the testosterone dose. Less substrate means less estradiol. This is often the cleanest fix.
- Split injections. Smaller, more frequent doses produce lower peaks and less conversion.
- Lose body fat. Less fat tissue means less aromatase.
- Limit alcohol, which raises aromatase activity and stresses the liver that clears estrogen.
Aromatase inhibitors: how they are used medically
Aromatase inhibitors (AIs) block conversion. They are approved in Canada for breast cancer in women:
The common mistake is taking an AI “just in case” and crashing estradiol, then blaming testosterone for the joint pain, low libido and flat mood that follow. Exemestane is irreversible (“suicidal”), so its effect lasts until the body makes new enzyme; anastrozole and letrozole are reversible. Letrozole is very strong and easy to overdo.

What about gynecomastia?
If you notice a tender lump under the nipple, act early. Early gynecomastia can often be reversed when the cause is addressed. Doctors sometimes use tamoxifen off-label for this; see our Nolvadex guide. Long-standing glandular tissue usually does not shrink with medication and may need surgery. Any breast lump in a man should be checked by a doctor.
Frequently asked questions
Do I need an AI on testosterone?
Not automatically. Many men on moderate doses never need one. Use bloodwork and symptoms, and try lowering the dose or splitting injections first.
What estradiol level is normal for men in Canada?
Ranges vary by lab, but many adult men fall around 70–150 pmol/L. Use a sensitive test and compare with your own baseline.
What are signs of low estrogen in men?
Joint pain, low libido, weak erections, fatigue, low mood and dry skin are the most common.
