How it works
By activating GLP-1 and GIP receptors it reduces appetite, slows stomach emptying and improves insulin release when blood sugar is high. A half-life of about 5 days allows weekly dosing. In trials, average weight loss was around 15–21%.
Half-life: about 5 days.
Release profile
How levels rise and fall after a single dose, based on a half-life of about 5 days. Each mark on the axis is one half-life; after about five, almost none is left.
An illustration from the half-life only; real levels vary with dose, injection site and the person.
Clinical reference dose
Clinical reference: the labelled starting dose for this class is 2.5 mg once a week for 4 weeks, then 5 mg, increasing by 2.5 mg every 4 weeks as needed to a maximum of 15 mg a week.
Mixing: adding 1 ml of bacteriostatic water to this 10 mg vial gives 10 mg per ml. On a U-100 insulin syringe, 10 units (0.1 ml) then holds 1 mg.
Step up slowly: stomach side effects peak after each increase.
These are reference amounts from labels and clinical studies, not a personal recommendation. A doctor sets the right dose for you, using your bloodwork.

Combinations and interactions
Often compared or used with
- Compared with semaglutide (SM5), which acts on GLP-1 only, and retatrutide, which adds glucagon.
- Never combine two GLP-1-type medicines.
- High protein and strength training help keep muscle while weight drops.
Interactions to know
- Insulin and sulfonylureas: low blood sugar risk.
- Birth control pills: absorption can drop after starting and after each increase; use a backup method for 4 weeks.
- Oral medicines: slower stomach emptying changes absorption.
Tell your doctor and pharmacist about everything you take, including supplements, before adding anything new.
