Adding SARMs to an anabolic steroid course is common online, but there is no human research on the combination. SARMs are weaker than steroids, so their extra contribution is usually small, while their effects on cholesterol, the liver and natural testosterone add to what the steroids already do.
- SARMs and steroids act on the same androgen receptor.
- Side effects stack, especially lower HDL and suppression.
- If something goes wrong, it is hard to know which compound caused it.
The idea sounds logical: steroids for the base, a SARM for a little extra. In practice, the combination brings more questions than answers. This guide explains how the two classes compare, what the combination adds, and why most harm-reduction advice is cautious about it.
Same receptor, different strength
Selective androgen receptor modulators (SARMs) and anabolic steroids both work by activating the androgen receptor, the same switch testosterone uses. SARMs were designed to act more on muscle and bone and less on the prostate and skin. In clinical trials they produced modest gains in lean mass: about 1–1.5 kg over several months for ostarine, and about 1.2 kg in three weeks for LGD-4033 at its highest tested dose. Steroids such as testosterone produce much larger changes.
When testosterone levels are already well above normal, most androgen receptors in muscle are already busy. A SARM added on top has fewer free receptors to act on, which is one reason users often report little noticeable difference.
| Anabolic steroids | SARMs | |
|---|---|---|
| Receptor | Androgen receptor (plus estrogen via conversion for some) | Androgen receptor, more tissue-selective |
| Strength | Strong | Modest |
| Human data | Decades of medical use | Short trials, none approved |
| Suppress natural testosterone | Yes | Yes, dose-dependent |
| Lower HDL cholesterol | Yes | Yes, often strongly |
Our SARMs 101 guide and SARMs vs steroids comparison cover the basics.
Why people combine them
- To add a “dry” compound without the liver strain of an oral steroid
- To try a compound perceived as milder
- Because SARMs are easy to buy as research chemicals
Some of these assumptions are shaky. SARMs are not free of liver risk: case reports in medical journals describe liver injury with LGD-4033 and RAD-140. And they are not milder on cholesterol: in trials, HDL dropped clearly within weeks.
How the side effects stack
Cholesterol
Both classes lower HDL (“good”) cholesterol. Combined, HDL can fall to very low levels, raising long-term heart risk. See steroids and cholesterol.
Liver
Adding a SARM to an oral steroid doubles the load on the liver. Even with injectables only, SARM-related liver injury has been reported.
Suppression and recovery
SARMs suppress natural testosterone on their own; on top of steroids, they add to the time natural production is switched off.
Vision and other specific effects
Some SARMs have their own problems, such as the vision changes reported with S4 (andarine). See S4 and vision.
Harder troubleshooting
With two or more compounds, a side effect such as low libido, acne or raised liver enzymes could come from either. That makes it harder to decide what to stop.
Product quality
A 2017 study in JAMA tested 44 products sold online as SARMs. Only about half contained a SARM at all, many contained other unapproved drugs, and a quarter contained substances not on the label. Adding a mislabelled product to a steroid course adds unknown risk. Choose suppliers that publish batch-specific laboratory results; see liquid vs capsule SARMs for what to look for.
Bloodwork
- Lipid panel
- Liver enzymes (ALT, AST, GGT) and bilirubin
- Complete blood count
- Testosterone, estradiol, LH and FSH
- Kidney function
Test before starting and a few weeks in. Your doctor can order these, or you can pay privately at LifeLabs or Dynacare. See our bloodwork guide.
A simpler approach
If the goal is more progress, the usual levers are training, food and sleep before any extra compound. If a second compound is being considered, choose one with a well-documented profile and change one thing at a time; see a decision framework for adding a compound and the conservative approach.
Legal status in Canada
SARMs are not authorised by Health Canada for human use and are sold for research. Anabolic steroids are Schedule IV controlled substances. Both are banned in sport by the World Anti-Doping Agency.
Frequently asked questions
Can you stack SARMs with steroids?
People do, but there is no human research on the combination. SARMs add modest effects on top of steroids while stacking cholesterol, liver and suppression risks.
Are SARMs safer than steroids?
Not clearly. They lower HDL cholesterol, suppress testosterone and have been linked to liver injury, and many products are mislabelled.
Do SARMs add anything to a steroid course?
Often little that users notice, because steroids already occupy most of the androgen receptors SARMs would act on.
