Oral steroids are popular because they avoid needles, but most are harder on the liver and cholesterol than injectables. They range from mild (Anavar, Primobolan) to very harsh (Superdrol, Halotestin).
- Most orals are 17-alpha-alkylated, which lets them survive the liver but strains it.
- Orals lower HDL cholesterol more than injectable testosterone.
- Short use with liver and lipid bloodwork is the standard advice.
Tablets feel simpler and less intimidating than injections, which is why many people look at orals first. But “oral” does not mean “mild”. This guide compares the ten orals people ask about most, and explains why route matters for safety.
Why orals are different
Swallowed testosterone is almost completely broken down by the liver before reaching the bloodstream. Most oral steroids get around this with a methyl group at carbon 17 (17-alpha-alkylation). That makes them active as tablets, but the liver has to process a compound it cannot easily break down. Orals also cause bigger drops in HDL (“good”) cholesterol than injectable testosterone. Our guide to oral bioavailability explains the science, and methylated steroids explained covers the liver side.
The ten compared
| Oral | Compound | Liver strain | Known for |
|---|---|---|---|
| Anavar | Oxandrolone | Lower | Lean tissue, little water; strong HDL drop |
| Primobolan (oral) | Methenolone acetate | Low (not 17-alkylated) | Mild, slow |
| Proviron | Mesterolone | Low (not 17-alkylated) | Androgenic, weak anabolic |
| Andriol | Testosterone undecanoate | Low (absorbed through lymph) | Medical TRT tablet |
| Turinabol | Chlorodehydromethyltestosterone | Moderate | Dry gains, East German doping history |
| Winstrol | Stanozolol | Moderate to high | Dry look; joint and cholesterol issues |
| Dianabol | Methandrostenolone | High | Fast size, water, estrogen effects |
| Anadrol | Oxymetholone | High | Fast weight and strength, blood pressure |
| Halotestin | Fluoxymesterone | Very high | Strength, aggression, little size |
| Superdrol | Methasterone | Very high | Very potent; cases of liver injury |
Medical doses where they exist
Several of these orals were, or still are, prescription drugs. Their label doses show how small medical amounts are:
The mildest and the harshest
Anavar is often called the mildest 17-alkylated oral and was studied in burns, HIV wasting and older adults. It still lowers HDL substantially. Primobolan and Proviron are not 17-alkylated and are gentler on the liver, but also weaker.
At the other end, Superdrol has been linked to published cases of severe cholestatic liver injury, and Halotestin is extremely androgenic with marked cholesterol and mood effects. Read our guides to Superdrol and Halotestin before considering either.
Side effects common to orals
- Raised liver enzymes; cholestasis with harsher compounds
- Large drops in HDL cholesterol and higher LDL
- Higher blood pressure
- Suppression of natural testosterone
- Acne and hair loss with the more androgenic ones
Oral or injectable?
Many people choose tablets to avoid needles, but from a health point of view injectable testosterone is usually the gentler option. It is the hormone your body already makes, it does not need to be 17-alpha-alkylated, and its effect on cholesterol is smaller than most orals. Orals are also short-acting, so blood levels rise and fall each day. If injection anxiety is the main reason for choosing tablets, it is worth knowing that small insulin-style needles and good technique make injections far less unpleasant than most people expect; read overcoming injection anxiety and how to inject safely. Our injectables use thin MCT oil, which passes easily through smaller needles.
Bloodwork for orals
Liver enzymes (ALT, AST, GGT, bilirubin) and a lipid panel are the priorities, along with a complete blood count and kidney function. Test before starting and within three to four weeks. Note that heavy training also raises ALT and AST, so mention your training to whoever reads the results. See our bloodwork guide and liver support during orals.
Legal status in Canada
Anabolic steroids, including all of these orals, are Schedule IV controlled substances under the Controlled Drugs and Substances Act and are banned in sport. For the legal details, read is it legal to buy steroids in Canada.
Frequently asked questions
What is the mildest oral steroid?
Among 17-alpha-alkylated orals, oxandrolone (Anavar) is usually considered the mildest. Primobolan and Proviron are not 17-alkylated and are gentler on the liver but weaker.
Are oral steroids worse than injections?
For the liver and cholesterol, generally yes. Most orals strain the liver and lower HDL more than injectable testosterone.
Can you take oral steroids without testosterone?
Orals suppress natural testosterone, so using them alone often causes low-testosterone symptoms. This is why they are usually discussed alongside testosterone.
